It is very important that your pet’s primary veterinarian know what treatment and medications your pet is receiving from us. Please provide the contact information of the veterinarian you would like to receive these progress notes.


Appointment and Cancellation Policy

 

Our relationship with you is built on trust and consideration. We’re committed to punctuality and ask that you reach out if you can’t make your appointment. We send reminders via text, email, and phone calls, and now REQUIRE confirmation for all appointments. Please be mindful of the impact missed or unnotified cancellations have on patients in need of essential care. Below are our policies for missed appointments:

 

Late Arrivals: If you arrive 10 minutes or more after your designated appointment time, we will need to reschedule your visit. Rushing through or squeezing in appointments compromises the quality of care we strive to provide.

Appointment Cancellation: Notify us 48 hours before your appointment to respect the needs of other patients. If you need to cancel after office hours, you may leave a voicemail at 239-948-9434, respond to one of our text or email reminders.

Missed Appointments (No Show): Missing an appointment without 48-hours notice will be noted in your pet’s record and considered a No Show. A $100 deposit is required for future appointments, applied to the next visit. This deposit is forfeited if the rescheduled appointment is also a “no-show.”

 

These policies help us accommodate as many patients as possible. Thank you for your understanding.

 

Animal Dermatology & Allergy specializes in the treatment of allergies, ears, and skin diseases only. Your primary care veterinarian will continue to provide any other medical needs of your pet. Animal Dermatology & Allergy does not provide after-hours emergency care. In case of a medical emergency, you should contact your primary care veterinarian or an emergency veterinary facility in your area.

 

Payment is due at the time of service. The fees charged to you today cover only today’s visit. Future services and medications will be invoiced separately. We accept cash, personal checks, MasterCard, Visa, Discover, and American Express.
