Referring a Patient

To refer a patient have the owner call to schedule an appointment. Should you need more brochures or cards to give to your clients please call (239) 948-9434  and we will mail those to you immediately.

Referral Forms

To allow us to have a complete history please send 2 years, if available, of medical records along with any bloodwork or previous diagnostics the patient has had performed.

Once a patient has been seen at our clinic every effort is made to contact you within 48 hours of the examination. Typically, we fax a referral letter at the end of the day. If you would prefer a phone call, email or mailed letter please let us know and we are happy to accommodate your wishes.
